August Bank Holiday Opening Times

Reopening Tuesday 27th August at 8.30am 

During the times we are closed if you have a life threatening medical emergency please hang up and dial 999.

If you need to see a doctor urgently please ring Badger - 0121 766 2100

If you require advice or information whilst we are closed please dial 111. The advice and information service is available 24hrs a day. 

Our normal opening hours are Monday to Friday 8.30am to 6.30pm. The practice is closed between 1.00 and 3.00pm on a Wednesday and Thursday afternoon at 1.00pm.

New Patient Registration

If you are normally resident within our catchment area and would like to register with the practice please ask a member of our reception team for a new patient registration form or download one here.

You will be asked to complete a New Patient Questionnaire about your personal background and health, along with provide a form of identification, i.e. passport or drivers licence, and proof of address. We plan to process this information under the Data Protection Act and will maintain your confidentiality at all times.

All new patients will be booked an appointment with one of our nurses for a new patient health check when they register.

If you are registering a child under six and a half years old it is important that we have their immunisation details as part of the registration process. For example, suitable evidence is contained in the child’s RED BOOK. Please bring this with you.

Updating Your Details

If you change name, address or telephone number, please let our receptionists know by calling or writing to us. Alternatively you can complete the online form. If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
